Creating Online Surveys/Feedback Forms

The Internet will give you plenty of options in gathering your audience feedback.

Google offers one of the easiest ways to gather user feedback. Google forms allow the audience to answer a set of questions you have set. These can be used for surveys, feedback, online registrations, and customer care support.

These are the following step in creating feedback forms:

Step 1: Open your browser and go to drive.google.com

Step 2: Sign in or create account

Step 3: On the left-hand side of your My Drive page, click New>More>Google Forms

Step 4: You will be taken to Google Form's Interface:

Step 5: You may now fill out the form with questions:

        a. Question Title - Includes the question that will be answered by your audience

        b. Help text - Creates a subtext under the question to give more information about a question

        c. Question type - Changes the type of question according to your preference.
           These option include:

                 i. Text - A question that can be answered in a short text

                 ii. Paragraph Text - A question that can be answered in a long text

                 iii. Multiple Choice - A question that can be answered by only one answer in a set of options

                 iv. Checkboxes - A question that can be answered with multiple answer in a set of options

                 v. Choose from a List - Similar to a multiple-choice question but the options are revealed in a drop d down list

                 vi. Scale - A question that can be answered with a numerical range

                 vii. Grid - A question that contains sub-questions with similar options as shown:

      d. Advanced Settings - Contains more options for the specified question type

Step 6: Once you are done editing the question, Click done

Step 7: To add a new question or item, click on Add Item. Tip: Clicking on the drop-down arrow at the right side of the Add Item button will bring up other options for layout:

      a. Section Header - Adds a header for a specific part of your survey

      b. Page Break - Adds a page break which means that the items at the button of the page will be added to the next page; necessary if your survey is too long

      c. Image - Adds an image to your survey

      d. Video - Adds a video to your survey

Step 8: Edit the Confirmation Page option at the bottom as you see fit

      a. Show link to submit another response -  Allows the users to answer the same form again

      b. Publish and show a public link to form results - Allows users to see the summarized results for your survey

      c. Allow respondents to edit responses after submitting - Allows users to go back and edit their answers.

Step 9: Click the Send Form button. The Send Form dialog box will appear. Edit these options as you see fit.

     a. Link to share - Contains the URL that you can share on your website

     b. Embed - Contains an embed code to attach to your HTML

     c. Short URL - Generates a shorter URL necessary for limited spaces like sharing on twitter

     d. Share link via - Allows you to share the link to Google+, Facebook, and Twitter

     e. Send from via email - Allows you to share the form via email

     f. Add collaborators - Allows others to edit your form, necessary when working with groups

      I will give 5 awesome benefits that we can get by using the Google Form. So let's start;
      First, you can create unlimited free forms because most survey tools online will only offer a certain amount of free polls or surveys before they start charging you for the service. Google, will let you run as many polls and surveys as you like and you can just keep creating new Google Forms until your Google Drive is full. You’ve also got a lot of variety in the types of forms that you can build. You can use all sorts of question types and use a variety of themes on your form.
      Second, the results can go directly into a shareable spreadsheet, if you love collaborating on things with friends or co-workers, then you’ll appreciate that Google Forms will allow you to share both editing the forms and the results. Everyone can view them at the same time, make edit at the same time and see other people’s edits as they make them.
      Third, is the logic threading ability to change the survey based on the answers the participant has given. For instance, if the first question of the survey asks if you are a Mac or Windows user, the following questions can be based on Mac or Windows accordingly.
      Fourth, is that you can get email notification of results because some of the poll-making and survey-making tools available online will insist that you revisit their site in order to check on results and this is not a user-friendly in my opinion.
      Lastly, forms are mobile-friendly Not all web pages are created equally. Some are best displayed on a desktop machine, and some require Java or Flash. Not all of the readily available free survey-making tools are great on a mobile browser. But Google Forms are. All Google Forms are able to be used with any simple browser on any device.

    1. The disadvantages of Google Forms:
      For instance, the teacher are assigning a form to a multiple Classroom Classes, it will create multiple copies of the forms and the results sheet, this means that the teacher need to check it one by one for which the work is assigned. Also, when copying the form it creates a new results sheet and does not copy the Add-Ons and Settings. Also there are limitations that a Google Form have like it can be completed many times as a person wants to access the survey, no ability to format text and lastly it's hard to move the questions other than up or down. They are all flush left. So, I hope you learn something.

    1. You can use validation that will make the respondents to be serious because if there not going to be serious on there response they won't be able to submit it. For example, you will make the school ID number, age, and school required. So they will answer truthfully.

      First, it Can be completed as many times as a person wants to access the survey.
      Second, no ability to format text.
      Third, it's hard to move questions other than up or down. They are all flush left.

      Instead of taking the time to view and sort the Google sheet, teachers can just look at the Done vs. Not Done, which updates live as students submit their Forms. This works just like the submission of any other assignments on Classroom.

      If there are multiple files added to the Assignment that includes the form, students will need to click Mark as done manually, otherwise this is now automatic.

      Disadvantages:
      Instead of assigning the live form link to the assignment, it is actually creating a copy of the form itself within Classroom, just like the Make a Copy option would for any other Google doc. The problem here lies in multiple classes: if you are assigning a form to multiple Classroom classes, it will create multiple copies of the form AND the results sheet. This means that the teacher has to check responses for each class section for which the work as assigned. The ease of the automatic Mark as done probably outweighs this extra step for most teachers, though, making the feature still worthwhile. Unless you love Add-ons.
      When you use Add-ons to manipulate data or communicate with users, it often takes a while to program effectively.

    1. Yes you can gather feedbacks from social media but using forms it is much easier because assure that the respondents will give their profile as you wish to. You can also filter their answers using validation with the use of the feedbacks forms and for me it is easier to conduct surveys using feedbacks form rather than other social media. That's all.     1. Google forms is a cheaper and free way to add a signup page to your FB fan page. Google forms give you an option of capturing user inputs and storing it into a backend spreadsheet.

      Lastly, You can compare results. Surveys results provide a snapshot of the attitudes and behaviors – including thoughts, opinions, and comments – about your target survey population. This valuable feedback is your baseline to measure and establish a benchmark from which to compare results over time.
